ACTE DE NOTORIÉTÉ

The Haitian Consulate offers the Acte de Notoriété service to eligible applicants seeking official recognition of identity or civil status. Applicants must appear in person accompanied by two (2) witnesses.

Required Documents

Birth certificate (copy)

A copy of a recent Archival Extract (2001 or more recent)

A valid American ID (ID/USA)

Two (2) witnesses, each carrying

Their valid Haitian passport (original)

A valid state ID

Important

Please note that this ID must contain the applicant's current address and an ID number

Applicants must appear in person at the Consulate accompanied by two (2) witnesses and present all required documentation as listed above

Remarks

All documents must be presented in their original form unless otherwise specified. Incomplete or missing documentation will result in the appointment being rescheduled. Please verify all documents are valid before your scheduled visit.
