BODY REPATRIATION

Necessary requirements to facilitate the transfer of human remains to Haiti.

Required Documents

  • Death Certificate (original)

  • The passport of the deceased

  • The Transit Permit

  • The Burial or Removal Permit

  • The Embalmment Certificate (This certificate must be notarized, then legalized by the County Clerk)

  • Copies of both Funeral home and Director Licenses

  • The Original Copy of the Funeral Home bill

  • The Name, Address, and telephone number of the Funeral Home receiving the body in Haiti

  • The Date of departure, Name and flight number of airline transporting the body to Haiti

Important Notice

  • Please be advised that all documents must be verified and legalized at the Consulate before the shipment to Haiti.
