For Haitian citizens renewing an expired or soon-to-expired passport
Required Documents
For the Applicant:
US birth certificate
Notarized French translation of the birth certificate
Two (2) passport-sized photos
Extract from the Archives (issued after 2001)
For Minors:
Signed parental authorization form
Recent Extract from the National Archives (issued after 2001)
Legal custody documentation (if applicable)
Immigration document (e.g., visa, green card, etc.)
Important
Married women must bring a copy of their marriage certificate
Children under 2 years old: declaration made at the Consulate
Children over 2 years old: transcription required in Haiti via National Archives
