PASSPORTS RENEWAL - ALL APPLICANTS

For Haitian citizens renewing an expired or soon-to-expire passport

Required Documents

For Adults:

  • Valid U.S. State ID (not required for minors)

  • Copy of the ID page from the expired passport

  • Two (2) passport-sized photos

  • Extract from the Archives (issued after 2001)

For Minors:

  • Two Haitian witnesses with:

  • Parent/guardian's valid government ID

  • Parent/guardian's valid Haitian passport

  • Legal custody documentation (if applicable)

Important

  • Married women must bring a copy of their marriage certificate

  • Applicants without a recent Extract must bring a birth or baptism certificate to request one
